PHYSICAL THERAPY EVALUATION
An evaluation includes review of current medical history, physical impairments, or functional limitations, and allows you to work with the therapist to design, implement, and modify therapeutic interventions. Some areas of assessment may include range of motion, flexibility, strength, posture, balance, body mechanics, and work, hobby, or sport demands. I may assess your breathing and ask questions about your emotions, beliefs, relationships, play, and responsibilities. You will be asked to complete a 2 to 3 page medical history form and to discuss or list your goals. Physical therapy does not include radiology or electro-surgery. I do not see Medicare or VA patients at this location and I do not bill medical insurance. The evaluation usually takes 1-2 hours and may or may not include PT treatment or instruction in a home exercise program. After completing an evaluation we will discuss follow up sessions to be scheduled in the future if needed.
The focus of a coaching session is directed by the client and encourages self-discovery. You are a whole individual responsible for your own choices. Making changes for better health and well-being can come from your own ability and resources but may be enhanced by the collaboration of a coach. Coaching does not take the place of medical care from a physician and is not counseling, psychotherapy, or mental health care. I am a Certified Health Education Specialist (CHES). A coaching session usually lasts 1-2 hours and we can schedule subsequent sessions if needed. I do not bill medical insurance for this service.
